How do other firms handle CT claims with multiple employers?
The injuries "card" only supports one employer and one carrier. Does one simply duplicate that card and put in other employers and carriers?
How do you record (or do you) the dates of liability / coverage for each defendant?
A question which has wider applicability: How do you keep track of the which adjuster/lawyer goes with which employer when preparing correspondence? - the parties list with which one is presented using the command does not differentiate between the various defense groupings.

If you mean for form letters, I can assure you that almost all of them are using the GetParty commands. Hopefully, other offices will start reading other threads and provide their input (also, in this way, everyone can learn from everyone else).

But, that's per injury. You may have as many injuries on one card as you like. I am thinking about increasing the carriers to two per injury. In the meantime, most offices just add a second injury and specify either the same dates of injury or the dates during the interim period for the CT.
