Application for Adjudication form

Is there any way that I can edit the Application so I can add some additional entry fields, and change some of the fonts.

It looks like the form itself was typed in Times New Roman, and my office would prefer it in Arial.

All of the merged data comes out in Courier -- which is perfect -- but some of the Yes/No fields have some strange font for the X, where as others down at the bottom are in Courier (as they should be).

Lastly, for paragraph number 1 of the Application, one line is often insufficient for all of the injured body parts. I want to add a field right after "...in the course of employment to," so there is a line and a half for body parts.

Please let me know how this can be done.

You may add user defined fields to each injury but if you mean on the application itself, let me know what you have in mind.

Do you mean they prefer the form in Arial or what they type when they fill in the application in Arial? The actual court form itself (provided by the WCAB) is in Times Roman.

I'll check it out. Do you have the latest version? The font style was recently changed.

This has been suggested and I am working on that.

I mean they prefer the form itself in Arial. We are used to using Legal Solutions, and that form is entirely in Arial. The text that is filled in should be entirely in Courier, but the form itself we want to be Arial.

I just realized we don't have the most recent version -- we're still on 4.1.0. I'll download your update and get back to you on the font style for the "YES/NO" Boxes.

I'm eagerly awaiting your update adding the field for the injured body parts.

Hello Gayna,

the form you faxed me appears to be printed from the Forms section in A1-Law. The application is the one form that you should print right from the Injuries tab at the client card (not the Forms window). Just click the printer icon in the window and you will see the Arial font.

Okay, I printed it out from the Injuries tab and with all of its problems, the one through forms is closer to what we want than this one.

1. The data is in Arial -- I want that in courier.

2. The form is still in Times New Roman -- that's what I want in Arial.

3. The App through "Injuries" still has "Y" and "N" for the yes/no boxes instead of X's.

Also I need all the entry fields that were indicated in my fax to make this form truly functional for us.

If we need a custom form for this that we need to go through "forms" to get, so be it.

this was done in a recent version. Tools, System, Configure System Defaults, Injuries, Use X instead of Y/N on printed apps.

Also, the apps now allow for twice as much room for the parts of body so there should be no problem with that either.

What item would be next for your "apps" priorities?

Hi Mike,

It's not a matter of "which item is next on my priority list" of things to change with the App. Do you remember the App I faxed over to you, with all of the extra entry fields I wrote on there, and the font changes? Until the App. is like that, with all of the fields and the font changes, and the X's for the injuries, and everything else I indicated, it's not usable by our office.

If it's got to be a custom Application for Adjudication just for us, so be it. This really is a big deal for us.
