I need to merge the client's name, employer & insurance company into a petition form I'm creating. It should look like this:

MARIA GOMEZ,
Applicant,
vs.

HYDRO-SCAPE PRODUCTS,
STATE COMPENSATION INSURANCE FUND,
Defendants,

However, I would need to see the codes you are currently using to enter the defendant (in upper and lower case) so that I can provide you with the codes to enter them in upper case.
