EAMS Forms- cover sheet, DOR

Hi Mike,

We are currently having problems with our Cover Sheet and DOR.
The problem with the Cover Sheet is that no dashes are merging between the SSN. Same thing with the DOR, but for the Phone Number. Can you help?
