Form #10455 - OCF SSA Appointment Of Representative (SSA-1696-U4)

Dear Michael,

Our Social Security secretary let me know that Form #10455 was having a box get automatically checked (Title VIII (SVB)) upon opening that she didn't want checked.

Reviewing the code it had the following:

fieldname = checkbox10,1

Removing this code solved the issue - Plus provided a tool for future letter modification.

Just wanted to bring to your attention.

Right - I think many prefer that checked as a default. You can use the:

courtform,debug1

at the top of the forms to figure out what each fieldname is called so that you can easily default information in any field. Most checkboxes use the 1=on and 0=off standard.
