OCR FORM AUTOPOPULATION BUG

We have just had some forms rejected by WCAB as there are a couple bugs in the auto-population of uniform names.

We have noticed a similar problem with two forms:

Application for Adjudication of Claim form (DWC/WCAB Form 1A, pg 2)
On pg 2 of the form (not counting the leading separator sheet) there are fields for employer, insurance carrier and claims administrator and so forth. The Insurance Carrier Name loads the Insurance Carrier office name correctly, but the Claims Administrator Name loads incorrectly. Instead of autopopulating this "name" field with the claims administrator firm (ie, the uniform name/location), the individual adjustor's name is inserted. I did doublecheck the EAMS website and it specifies "the claims administrator field gets the claims administrator’s office name", so having the individual adjustor's name inserted here is a bug. (For example, if the claims adjuster is Paul McCartney, working at BEATLES LIVERPOOL, we would want the claims administrator firm name BEATLES LIVERPOOL in the name field and instead we're getting Paul McCartney.)

Notice and Request for Allowance of Lien form (DWC/WCAB Form 6, pg 2)
On pg 2 of the form (not counting the leading separator sheet), the problem occurs in the field for the Employer or Claims Administrator Attorney/Representative Name (at the bottom of the form). Similar to the problem above, what gets auto-populated here is not the employer attorney FIRM NAME, but the individual attorney's name. (So for example, if the defense attorney is Davy Jones of MONKEYS LONDON, we are seeing the name Davy Jones filled in instead of the firm name MONKEYS LONDON.)

Normally we've tried to work around it and change the fields manually as we complete the forms, but when less experienced staffers missed it, the forms were rejected and returned. It would be very helpful if you could fix the autopopulation links to pull the firm names instead of the individual names for these forms.
