Can we create a subactivity that would correspond to each provider?
Thus, there would be, for example, five providers. Under each provider would be a chronological listing of reports.

You can enter the provider's name in the event and then easily filter off a specific provider. This way you only need one activity (Medical Reports) and you can easily print a specific provider's events in that category.
